As agreed upon by both the undersigned parties, the DJ service of “Anchorage Mobile Music” hereafter known as the DJ and _______________________________, hereafter known as the PURCHASER(S), agree to the following conditions:
1. Date event is to be held: Month _____ Day ____ Year ______
2. Location of event: ________________________________________
3. The DJ agrees to perform from ______ AM/PM to ______ AM/PM.
4. The DJ will be available to play until _______ AM/PM (Maximum).
5. For up to four hours of service $________Overtime services $_________ per hour.
6. The PURCHASER(S) agrees to pay DJ $_________deposit upon signing of contract to reserve time and date. Deposit is not refundable
8. The balance of $_________ is to be paid the day of the event in cash or cashier’s check.

9. The Reception Information Form is to be filled out, returned and discussed with the DJ at least ________________________ prior to the reception.
10. The DJ assumes no responsibility on the day of said event if the start time is delayed due to people’s late arrival or the establishment’s delayed meal service.
THIS CONTRACT IS LEGALLY BINDING AND IS SIGNED WITH THE UNDERSTANDING OF THE ABOVE CONDITIONS. I HEREBY AGREE TO THE ABOVE.
